THE HOCKEY FORMULA – PARTICIPANT WAIVER & RELEASE OF LIABILITY

Event: The Hockey Formula Camps & Skills Sessions

Participant Age: Under 18 (Parent or Legal Guardian must sign)

1. Assumption of Risk

I, the undersigned Parent/Guardian, acknowledge and fully understand that participation in ice hockey training, skating sessions, off-ice instruction, and related activities involves inherent and significant physical risks, including but not limited to: collisions, falls, puck or stick impact, equipment failure, ice conditions, and actions of other participants.

I understand that injuries can be minor, serious, catastrophic, or fatal.

I voluntarily allow my child to participate with full knowledge and acceptance of these risks.

2. Release of Liability

In consideration for my child’s participation in The Hockey Formula camp, I hereby release, waive, discharge, and hold harmless:

The Hockey Formula, Taylor Leier, all coaches, staff, volunteers, partners, facility owners, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, disability, property damage, or death that may occur during or in connection with the camp—whether caused by negligence or otherwise.

3. Facility & Rink Owner Release

I understand and acknowledge that The Hockey Formula conducts camps, skills sessions, and related activities at third-party facilities that are not owned or controlled by The Hockey Formula.

In consideration of my child’s participation, I hereby release, waive, discharge, and hold harmless the owners, operators, employees, agents, and representatives of all facilities used for The Hockey Formula activities, including but not limited to:
- Pilgrim Skating Arena in Hingham, MA
- The Bog in Kingston, MA
- Any other rink, arena, or training facility in which The Hockey Formula operates, skates, trains, or conducts programming from any and all claims, demands, actions, or causes of action arising out of or related to injury, loss, damage, or death occurring on or about the premises, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I acknowledge that ice rink facilities present inherent risks including ice conditions, boards, glass, locker rooms, common areas, and spectator spaces, and I voluntarily assume all such risks on behalf of myself and my child.

4. Medical Authorization

I authorize The Hockey Formula staff to:

I acknowledge that failure to follow safety instructions may increase risk and is the responsibility of the participant and parent/guardian.

I grant The Hockey Formula permission to photograph or record my child during the camp and use such media for promotional, marketing, or educational purposes without compensation.

8. Governing Law

This agreement shall be governed by the laws of the province/state in which the camp or session is held.
Any disputes shall be resolved through binding arbitration, not court litigation, to the fullest extent permitted by law.

9. Acknowledgment

I have read and understand this waiver.

I understand that by signing, I am giving up legal rights, including the right to sue The Hockey Formula or its representatives.

I sign voluntarily and without coercion.

10. Cancellation Policy

Out of respect for scheduling and ice time, cancellations made within 24 hours of the session require full payment.
