THE HOCKEY FORMULA – PARTICIPANT WAIVER & RELEASE OF LIABILITY

Participant Age: Under 18 (Parent or Legal Guardian must sign)

ASSUMPTION OF RISK

I understand that participation in hockey training activities, including shooting, stick handling, and off-ice drills, involves inherent risks of injury. These risks may include, but are not limited to, slips, falls, collisions, being struck by sticks or pucks, equipment failure, or other physical injuries.

I voluntarily accept all risks associated with participation in these training sessions.

RELEASE OF LIABILITY

In consideration for participating in these training sessions, I hereby release and hold harmless Taylor Leier, The Hockey Formula, and the property owner from any and all liability, claims, demands, damages, or causes of action related to injury, loss, or damage that may occur while participating in or attending these training sessions, to the fullest extent permitted by law.

PROPERTY ACKNOWLEDGMENT

I understand that these training sessions take place on private residential property. I agree to follow all safety instructions and accept responsibility for any damage to equipment, property, or surrounding areas caused by the participant.

MEDICAL CONSENT

In the event of an injury or emergency, I authorize the instructor to obtain necessary medical care for the participant if a parent/guardian cannot immediately be reached.

Photography & Media Release

I grant The Hockey Formula permission to photograph or record me or my player during the camp and use such media for promotional, marketing, or educational purposes without compensation.

Governing Law

This agreement shall be governed by the laws of MA in which the session is held.
Any disputes shall be resolved through binding arbitration, not court litigation, to the fullest extent permitted by law.

Acknowledgment

I have read and understand this waiver.

I understand that by signing, I am giving up legal rights, including the right to sue The Hockey Formula or its representatives.

I sign voluntarily and without coercion.

If the player is under 18, a parent or guardian will need to provide contact information and consent below before the player can participate in the session.
